It's not specifically for COD4. It's a quiz for my local LAN centre, and COD4 is among a bunch of ****ty prizes, so I though I'd enter.
That's Bahamut's Lagoon alright. My friend did nothing but play that on an emulator all through our 3D Modelling class a few years ago.

SNES. Not released outside of Japan, though there's fan translations.
